I will give you credit. For more ambitious than I am. I merely run/walk ok "the old man shuffle" for 4 miles 4 times a week. Knees can't handle much more. Next up: a purchased treadmill with a 40 degree incline. Push the heart and save wear and tear on the suffering knees.

Chuck if you would be interested in adding a highly effective minimal impact option for some higher intensity benefit, I have been having good results with REHIT on a CAROL bike. 4 miles 4x / week is excellent. I had great results with Platelet-Rich Plasma (PRP) Injections at HSS (https://www.hss.edu/condition-list_prp-injections.asp) with Jordan Metzl (https://www.hss.edu/physicians_metzl-jordan.asp). It took my chronic knee pain from 2-3 out of 10 to 0 out of 10. Hit me up if you want to discuss offline. That is a great incline on the treadmill. Something to consider if you want to up the intensity / reduce the time it takes for the workout -- a weight vest and/or a ruck for the treadmill. If you can get your knees fixed, that could increase the load that you can comfortably carry on the treadmill.

I should have added that the four miles takes about 40-48 minutes depending on the knees.

I will consider PRP. My wife recently got it in the shoulder...results TBD. I may contact you down the road. I will look at the websites.

I tore the miniscus off the bone on left leg. Slowly recovering and it should take me a year to get back to normal. Just missed having a knee replacement. After three months of doing rehab and going nuts I said enough and started the shuffle again against medical advice.

The treadmill was a Christmas gift. Got it on sale so spent around $3K all in. It may replace the "shuffle" because as you say the incline will push the heart without some of wear and tear. At least that is the plan. Really hope it works out. TBD as well. Currently prohibited from working on it by doc. Wife is aware and so I abide on this because I like domestic tranquility.

I like your posts. They mentally keep me moving forward which is a must since if I have anything to say about it, I won't be "checking out" until sometime after triple digits and day. :)
